HONOLULU – The Queen’s Health Systems (QHS) has named two new directors.
Gavin A. Sanjume, Pharm.D., BCPS, MBA, has been named Director of Retail and Contract Pharmacy Services.
Sanjume will provide leadership and direction to the Queen’s Physician Office Building pharmacies as well as the contract pharmacy at the Rehabilitation Hospital of the Pacific for The Queen’s Development Corporation.
Previously, Sanjume served as Medicare Part D STARs Pharmacist at United Healthcare. He has also served as a pharmacist at the Naval Health Clinic Hawaii, AlohaCare, the Kalihi Palama Health Center Downtown Clinic, Longs Drugs Stores and Castle Medical Center.
He currently serves in the United States Navy Reserve as a Medical Service Corps Officer.
He has a Master of Business Administration from the University of Massachusetts Amherst, a Doctor of Pharmacy from Purdue University and a Bachelor of Science in Pharmacy from Washington State University.
Hermina (Mia) Taylor RN, MSN, FNP-BC, APRN-RX, has been named Director of Community and Post-Acute Care Services.
Taylor will assist in the development of a system-wide continuum of care with an emphasis on the post-acute strategy and developing community programs and partnerships to improve patient access to high quality care and resources.
She previously served as manager of clinical programs at the Queen’s Clinically Integrated Physician Network (CIPN), focusing on developing community based clinical programs to help primary care physicians and specialists care for chronically ill high risk-high needs patients. Prior to joining Queen’s CIPN in 2015, she was a nurse practitioner with St Francis Hospice.
Taylor is a family nurse practitioner with 32 years of nursing experience in both inpatient and outpatient settings.
She received her Bachelor’s degree in psychology and sociology from Aquinas College in Grand Rapids Michigan and her Masters of Nursing from Michigan State University.
