Thank you for entrusting your patients’ care to the Queen’s Transplant Center.
Physicians may refer their patients with urgent liver transplant evaluation to one of our transplant hepatologists for an initial consultation by calling Physician’s Exchange 808-524-2575 (local) or 800-360-2575 (toll free); and nonurgent liver transplant evaluation or management of chronic liver disease to Queen’s Liver Center by calling 808-691-8838.
Referrals to the Queen’s Transplant Center may be made by filling out the Kidney Transplant Referral Form. The patient should complete and sign the Patient Health Questionnaire for Kidney Transplant Evaluation. Fax forms directly to the Queen’s Transplant Center at 808-691-8896.

Prospective kidney donors are asked to complete a confidential, online health history questionnaire before coming in for an evaluation. Click here to complete the donor questionnaire.
If you are unable to access the questionnaire or want to learn more about living kidney donation, call 808-691-1120 to speak to our living donor coordinator, or fill out the Living Kidney Donor Intake Sheet and fax or mail the form directly to the Queen’s Transplant Center.
