For outpatient services, please submit electronic orders or fill out and fax the form below to 808-691-7007.

You may also work directly with our Patient Service Coordinators who are specialized in scheduling for specific modalities by calling 808-691-7171.

For your patient’s convenience, X-rays do not require an appointment. Once your order has been placed for this procedure, your patient can walk into any of our imaging centers during business hours without an appointment. For the quickest outpatient service, our locations at Physicians Office Buildings 2 and 3 offer convenient parking adjacent to the building and short wait times.
