Queen’s North Hawai‘i Community Hospital’s Diabetes Wellness Center offers diabetes wellness services to all members of our community. We provide comprehensive, interactive outpatient diabetes education, teaching patients how to successfully manage their diabetes and develop health practices that fit into everyday life.
The Diabetes Wellness Center provides patients with comprehensive and individualized care from our registered nurses, who are also certified diabetes educators. We incorporate evidence-based education, allowing patients to maintain their individual lifestyle while getting results in managing their health. Our caring team treats the spectrum of diabetes diagnoses from individuals at risk for diabetes, to those monitoring and maintaining ongoing diabetes, as well as those with gestational diabetes during pregnancy.
The Diabetes Wellness Center at Queen’s North Hawai‘i Community Hospital provides patients with comprehensive and individualized care. We provide the following services:
Our registered nurse-certified diabetes educator can provide blood-glucose monitoring utilizing a continuous blood-glucose monitor. Through a special infusion set administered to the patient, this small device monitors a patient’s blood sugar levels every five minutes for a continuous 72-hour period. The information gathered is provided to a physician to assist in determining an individualized plan of care for the patient.
The Gestational Diabetes Program is designed to help expecting mothers who have been diagnosed with gestational diabetes learn how to eat properly and monitor their blood sugar levels. Our registered nurse-certified diabetes educator remains in close contact with the patient and patient’s family throughout the pregnancy and after care.
Queen’s North Hawai‘i Community Hospital offers a comprehensive outpatient education program for patients who are newly diagnosed with diabetes or for those who need a diabetes management refresher. Topics include:
In addition, one-on-one consultations with our registered nurse-certified diabetes educator are available to patients for a more individualized approach.
The Outpatient Insulin Pump Program is a highly effective therapy that may significantly reduce the onset and progression of complications associated with diabetes. After a thorough assessment to determine if a patient is an appropriate candidate for the insulin pump, our registered nurse-certified diabetes educator will schedule an intensive “pre-pump” training. This is followed by a one-day insulin pump “startup” session that is coordinated with the diabetes care team, including a physician. The success rate of this program with our patients has been outstanding.
