Molokai General Hospital’s Patient Navigation Program, Kukui Ahi, helps patients, families and their caregivers navigate the health care system and facilitate screening, diagnosis and treatment. They assist with finding and getting access to valuable resources, support services and financial assistance that patients often need. Most importantly, they assist with coordination of air and ground transportation and lodging for patients requiring services and treatment on the outer islands.
Patient navigation services are tailored to the individual patient’s needs and provides culturally sensitive care. Patient navigators work collaboratively with multiple health care providers to promote communication and relationships with the patients. They also work closely with community organizations such as Moloka‘i Cancer Fund, Cancer Care, Pacific Cancer Foundation, Senior Aging Services and American Cancer Society to provide patients with valuable resources (nutritional supplements, financial aid, home care assistance, respite care, wigs, turbans, prosthetic bras, Look Good, Feel Better programs, etc).
Navigators also play an important role in coordination with The Queen’s Medical Center Survivorship Program and Cancer Support Groups. Our services have expanded to navigate patients with chronic and multiple diseases, emergency department assistance and follow-up as well as high-risk pregnancies.
