Advance directives are documents that state your choices about medical treatment or names someone to make decisions about your medical treatment if you are unable to make these decisions on your own. These documents are called “advance” directives because they are signed in advance to let your physician and other health care providers know your wishes concerning your medical care.
Advance health care directives can include documents such as a living will or power of attorney. If you have completed these documents, we will place a copy in your chart and records to ensure that your medical providers follow your instructions.
