Upon Arrival
Once you walk through our clinic doors, enter your first and last name at our registration kiosk and pull your printed ticket. Please proceed to one of our waiting rooms and have a seat. Our patient service representatives (PSR) are working hard to register patients in order of the time your appointment is scheduled. Once your appointment time approaches, the overhead system will announce your ticket number and the desk number. Please proceed to the desk with your ticket, and the PSR will assist you with checking in.
Patient Registration Process
All new patients will be registered with a medical record, a business account, and be asked to complete the necessary forms and provide us a written consent to treat prior to the start of any service.
A check-in is required prior to every visit and you will be asked to verify and or update your information each time you come in. This ensures that the information (name change, phone number, mailing address, insurance) in our system is current, up to date, and accurate. We kindly ask for your understanding and appreciate your patience and cooperation during this process.
